Submitting the following forms will incur annual payments automatically through PayPal.
You must have a PayPal account so that subscription(s) can be set up. If you don’t have one you can create one after you submit a form.
All payments will be made yearly on July 1.
You may cancel automatic payment for membership fees or donations at any time by accessing your PayPal account and cancelling the automatic payment(s).
